Evaluation of the effectiveness of hospital infection prevention and control based on the SMART principle.

Hospital infection not only increases patient suffering and mortality, but also causes huge economic losses. It is urgent to take effective measures to improve the situation of hospital infection and focus on hand hygiene and disinfection. The SMART principle is to follow the principles of specificity, measurability, attainability, relevance, and timeliness when setting goals. This study aims to use the SMART principle in the prevention and control of hospital infection, to take a series of measures on the hospital infection such as hand hygiene, and to evaluate the implementation effect. This study is a retrospective study. From January to June 2020, the SMART principle was used to carry out intervention for infection prevention and control in the Xiangya International Medical Department, Central South University, and implement the infection prevention and control quality improvement plan. Using indicator data as targeting measurement standard, we established evaluation indicators for hospital infection prevention and control knowledge awareness and hand hygiene compliance rate as the process monitoring, surgical site infection rate and catheter-related urinary tract infection rate as the result monitoring. The evaluation indicators after intervention (July 2020 to June 2021) were compared with those before intervention (January 2019 to December 2019). Fifty-one medical staff working in the Xiangya International Medical Department were included. There were 12 doctors and 39 nurses, and 6 males and 45 females. The age ranged from 21 to 57 (30.2±7.1) years. After the intervention, the awareness of hospital infection prevention and control knowledge and hand hygiene compliance rate of medical staff were significantly higher than those before the intervention (both P<0.05), but there were no significant differences in surgical site infection rate and catheter-related urinary tract infection rate before and after intervention (both P>0.05). After the intervention, the awareness of hospital infection prevention and control knowledge and hand hygiene compliance rate of medical staff in the internal and surgical wards were higher than those before the intervention (all P<0.05). The awareness of hospital infection prevention and control knowledge of medical staff in surgical wards was low but improved significantly. The compliance rate of hand hygiene of medical staff in internal wards was high and improved significantly. After the intervention, the awareness of hospital infection prevention and control knowledge and hand hygiene compliance rate of male and female medical staff were higher than those before the intervention (all P<0.05), both of which were significantly higher in males than in females. After the intervention, the awareness of hospital infection prevention and control knowledge and hand hygiene compliance rate for medical staff aged less than 30 years old and 30 to 40 years old were higher than those before the intervention (all P<0.05). Medical staff aged 30 to 40 years had a low awareness of hospital infection prevention and control knowledge, but both indicators improved most significantly. After the intervention, the awareness of hospital infection prevention and control knowledge of doctors and nurses was increased (both P<0.05). The awareness of doctors improved more significantly, and the hand hygiene compliance rate of nurses improved (P<0.05). After the intervention, the awareness of hospital infection prevention and control knowledge and hand hygiene compliance rate of medical staff with primary and intermediate professional titles were higher than those before the intervention (all P<0.05), and the improvement was more significant for medical staff with intermediate professional titles. Based on the SMART principle management method, the implementation of quality improvement plans combined with various domestic and international evaluation standards for infection prevention and control can effectively improve the awareness of infection control knowledge and hand hygiene compliance rate of medical staff, strengthen the prevention and control of hospital infection, and further ensure the safety of patients.

Effect of Nursing in Operating Room Combined with Intraoperative Heat Preservation Intervention on Prevention of Incision Infection and Improvement of Hemodynamics in Patients with Anterior Cruciate Ligament Injury and Reconstruction under Knee Arthroscopy.

Objective To explore the effect of nursing in operating room combined with intraoperative heat preservation intervention on preventing incision infection and improving hemodynamics in patients with anterior cruciate ligament (ACL) injury and reconstruction under knee arthroscopy. Methods About 200 patients with knee arthroscopic ACL reconstruction in our hospital from January 2019 to July 2021 were enrolled. The patients were randomly assigned into two groups: the control group and the study group. The former group received nursing care in the operating room operating room and the latter group received nursing care in operating room combined with intraoperative heat preservation intervention. Nursing satisfaction, incidence of incision infection, knee joint VAS score, knee joint range of motion, knee joint Lysholm score, and hemodynamic indexes were compared. Results First of all, we compared the nursing satisfaction, the study group was very satisfied in 78 cases, satisfactory in 20 cases, and general in 2 cases, and the satisfaction rate was 100.00%, while in the control group, 445 cases were very satisfied, 20 cases were satisfied, 15 cases were general, and 8 cases were dissatisfied. The satisfaction rate was 82.00%. The nursing satisfaction of the study group was higher compared to the control group (P < 0.05). Secondly, we compared the incidence of incision infection. The incidence of incision infection in the study group was lower compared to the control group (P < 0.05). With regard to the knee joint VAS score, the knee joint VAS score of the study group was lower compared to the control group at 2 weeks, 4 weeks, 8 weeks, and 12 weeks after operation (P < 0.05). In terms of the range of motion of the knee joint, the range of motion of the knee joint in the study group was higher compared to the control group at 2 weeks, 4 weeks, 8 weeks, and 12 weeks after operation (P < 0.05). Regarding the knee joint Lysholm score, the knee joint Lysholm score of the study group was higher compared to the control group at 2 weeks, 4 weeks, 8 weeks, and 12 weeks after operation (P < 0.05). Finally, we compared the hemodynamic indexes. Before nursing, there exhibited no significant difference (P > 0.05). During and after nursing, the indexes of HR and MAP in the study group fluctuated little (P < 0.05). Conclusion During the perioperative period of patients with ACL injury and reconstruction under knee arthroscopy, standardized and necessary operating room combined with intraoperative thermal insulation intervention measures should be given, attention should be paid to the management of operating room, and intraoperative thermal insulation intervention should be strengthened. It includes preoperative visit, psychological nursing of patients, strict application of antibiotics before operation, monitoring of air quality in operating room, disinfection and sterilization of surgical instruments, shortening operation time, maintaining body temperature during operation, and paying attention to hand hygiene of medical staff. It plays a supervisory role in promoting the attention of medical staff to the prevention of wound infection, which is beneficial to the healing of surgical wounds of patients. It plays a positive role in enhancing hemodynamic indexes. Comprehensive nursing intervention on the risk factors of each link can effectively prevent postoperative wound infection and strengthen the prognosis and quality of life of patients.

Modern approaches to ensure hand hygiene of medical staff in a measure complex for prevention of nosocomial infections in health facilities

Hand hygiene of medical staff plays a significant role in the prevention of nosocomial infections in healthcare facilities. Therefore, there is a need to identify approaches to facilitate proper adherence to hand hygiene. The study aims to substantiate the need for hand hygiene of medical staff, to explore modern methods for evaluating the effectiveness of hand hygiene supplies and problems associated with the use of these supplies along with their solutions. The authors have carried out the general and targeted analysis and expert evaluation of research results regarding measures for the prevention of nosocomial infections in health facilities. Analytical study of a large number of sources (over 50) on modern approaches to ensure hand hygiene of medical staff has been conducted with Content analysis and Open Source Intelligence methods. As a result of the study, authors have identified the need for further development of strategies to facilitate hand hygiene in health facilities and for the improvement of existing educational and motivational programs dedicated to hand hygiene of medical staff. Factors that may affect the effectiveness of antiseptics usage have been highlighted. The authors emphasize adverse reactions associated with the use of hand hygiene supplies (contact dermatitis and contact allergy) and the need for the development of measures to reduce risks of these adverse reactions among medical staff.

[Practice of nosocomial infection management in burn department based on the American hospital evaluation standard of the Joint Commission International].

Objective: To explore the role of continuous quality improvement measures based on the American hospital evaluation standard of the Joint Commission International (JCI) in prevention and control of nosocomial infection in Burn Department of the Second Affiliated Hospital of Zhejiang University School of Medicine (hereinafter referred to as the author' s department). Methods: From 2013 to 2018, based on 11 JCI standards related to infection prevention and control and the current situation of the author' s department, more than 50 doctors, nurses, and nursing assistants from the author' s department participated in continuous improvement of the three-level management system of nosocomial infection in the author' s department, focusing on implementing of management of patient with multidrug resistant bacteria infection, optimizing the infection control management of instrument and cloth, and implementing target management on 5 indicators such as hand hygiene implementation rate, and carrying out inspection, quality management, and improvement on 11 items of prevention and control of nosocomial infection. The implementation rate of hand hygiene from 2013 to 2018 and the accuracy rate of hand hygiene from 2016 to 2018 of medical staff in the author' s department, and incidences of catheter-related bloodstream infection (CRBSI) of central venous, catheter-associated urinary tract infection (CAUTI), and ventilator associated pneumonia (VAP) of burn intensive care unit in the author's department from 2013 to 2018 were monitored.The following 7 indicators were monitored from 2013 to 2018, including false negative rate of nosocomial infection, incidence of hyperglycemia during intensive insulin treatment for severely burned patients, the implementation rate of CRBSI preventive measures, the specification rate of surface fixation of indwelling catheter, the implementation rate of VAP preventive measures, the accuracy rate of bed temperature during the use of suspended bed, and the implementation rate of hand hygiene of standardized training medical staff in the author' s department before and after improvement. Data were statistically analyzed with chi-square test. Results: The implementation rate of hand hygiene of medical staff in the author' s department was 88.0%-89.5% from 2013 to 2018, the correct rate of hand hygiene of medical staff in the author' s department was 95.10%-97.35%, and both reached the target values. The incidences of CRBSI in 2015, VAP in 2017, and CAUTI in 2013, 2014, and 2017 of burn intensive care unit failed to reach the respective target value and reached the respective target value after quality improvement, and the above-mentioned 3 indicators reached the respective target value in other years. From 2013 to 2018, the false negative rate of nosocomial infection and the incidence of hyperglycemia during intensive insulin treatment of severely burned patients in the author' s department after improvement were significantly lower than those before improvement (χ(2)=24.50, 4.74, P<0.05 or P<0.01), the implementation rate of CRBSI preventive measures, the specification rate of surface fixation of indwelling catheter, the implementation rate of VAP preventive measures, and the accuracy rate of bed temperature during the use of suspended bed after improvement in the author' s department were significantly higher than those before improvement (χ(2)=13.78, 6.50, 20.37, 13.92, P<0.05 or P<0.01), and the implementation rate of hand hygiene of standardized training medical staff in the author' s department after improvement was similar to that before improvement (χ(2)=1.71, P>0.05). Conclusions: The introduction of JCI standard can improve the implementation rate and accuracy rate of hand hygiene of medical staff in burn department, reduce the incidences of CRBSI, CAUTI, and VAP, and improve the effect of prevention and control of nosocomial infection in burn department.

Effect of Smart Hand Hygiene Management System on hand hygiene compliance of Intensive Care Unit medical staff

Objective To explore the effect of Smart Hand Hygiene Management System on hand hygiene compliance of Intensive Care Unit (ICU) medical staff. Methods Using convenience sampling method, 106 medical staff in the comprehensive ICU of Tianjin First Central Hospital were selected as the research objects. By deploying Smart Hand Hygiene Management System, medical staff's hand hygiene before and after contacts with patients in the ward were collected automatically, and compliance was calculated. By engineering means, the first month (July, 2018) data collected after removing the system's prompting function was taken as the baseline level, and the month after the system's acousto-optic prompting function was added and working principle training (August, 2018) was taken as the intervening stage. Compare the hand hygiene compliance of selected medical staff before and after intervention. Results The hand hygiene compliance of medical staff was 85.62% after intervention, which was higher than 52.87% before intervention and the difference was statistically significant (P<0.01) . After intervention, the pre-operation compliance increased to 73.90%, and the post-operation compliance increased to 94.94%, which were all statistically different from those before intervention (P<0.01) . Conclusions The application of Smart Hand Hygiene Management System has a significant effect on improving the compliance of hand hygiene performance of medical staff, and the initiatives of performing hand hygiene of medical staff have been greatly improved. Survey and analysis on the general surgery department medical staff hand hygiene compliance in a hospital

Objective To understand the hand hygiene condition and influence factors by researching, evaluating and analyzing the hand hygiene of medical personnel in the general surgery department of hospital. Methods A self-designed hand hygiene knowledge questionnaire was used to investigate 90 medical personnel of general surgery department of our hospital.Among them, there were 32 doctors and 58 nurses. Results 94.44%(85/90) of medical workers knew the seven-step method and 10.00%(9/90) believed wearing gloves can replace hand washing or hand disinfection.There were statistically significant differences in hand-washing rates between doctors and nurses before and after contact with patients and between medical and nursing operations (P<0.05). The average amount of bacteria before and after hand-washing by doctors and nurses was respectively (46.033±4.673)cfu/cm2, (36.281±3.029)cfu/cm2, (8.392±1.065)cfu/cm2 and(5.368±0.573)cfu/cm2. Conclusions By hand washing can effectively remove general surgery medical staff hand most of the bacteria, managers should strengthen the hand hygiene knowledge training and supervision, improve the quality of medical staff hand hygiene consciousness and prevention and control the occurrence of hospital infection. Hand hygiene intervention on general ICU acquired methicillin- resistant staphylococcus aureus infection and improvement

Objective To discuss the influence hand hygiene intervention on general ICU acquired methicillin- resistant staphylococcus aureus (MRSA) infection and its improvement. Methods Hand Hygiene Cognition Questionnaire that passed the reliability and validity test was used to compare the change of cognition on hand hygiene of medical staff. According to the results of the questionnaire, the intervention was carried out by continuous intensive training. The compliance with hand hygiene of medical staff was observed by monitoring equipment in the ward. Implementation status and effect of hand hygiene of medical staff on duty were examined randomly each month. At the same time MRSA infection rate of patients in comprehensive ICU was monitored in the same period. The relationship between hand hygiene compliance and MRSA infection rate was analyzed. Results The score of medical staff of cognition of hand hygiene was (41.70±3.67) points before the intervention, while the score was (44.10±3.55) points after the intervention. The difference had statistical significance (t=24.37, P <0.01). The correct rate of hand washing, positive rate of bacterial culture in hand, hand hygiene compliance and infection rate of MRSA of patients in comprehensive ICU were 68.75% , 14.58% , 66.90% , 12.90% respectively before the intervention. The correct rate of hand washing, positive rate of bacterial culture in hand, hand hygiene compliance and infection rate of MRSA of patients in comprehensive ICU were 88.54%, 3.12%, 74.14%, 3.10% respectively after the intervention. The difference had statistical significance (χ2=7.809- 24.520, P <0.01). Conclusions Questionnaires with high credibility reviews could better identify issues in hand hygiene compliance, and sustained, reinforcing intervention measures could improve the compliance of hand hygiene; Good hand hygiene practice of medical and nursing staff contributes to controlling MRSA infection rates in general ICU. Using feedback with adenosine triphosphate (ATP) bioluminescence assay improved the cleanliness of computerized nursing cart

Purpose: The Centers for Disease Control (CDC) recommends hand hygiene as the simplest, most effective method to prevent health care-associated infections. The purpose of this study was to review of literature on effectiveness with hand hygiene strategies in Taiwan medical institution. Methods: A review for relevant published literature in 2004-2014 was undertaken using electronic databases. Databases searched included: Medline, CINAHL, EMBASE, Chinese electronic periodical Service (CEPS), National Digital Library of Theses and Dissertations in Taiwan. Eight studies met the inclusion criteria. Results: Results showed that 8 hand hygiene programs were effective. Hand hygiene strategies such as:Use of alcohol-based hand rubs at the points of patient care. Education program, posters, slogans, revised hand hygiene standard, online self-learning materials, established a monitoring mechanism and enforcement of reward-punishment regulations. To improvement of hand hygiene compliance can effectively transform to the healthcare quality and patient safety in medical centers. Conclusions: These review shows that hand hygiene strategies can increase the medical staff hand hygiene compliance rate and the correction rate of hand hygiene.

The effect of high-quality nursing management in the control of hospital infects

Objective To explore the clinical effects of high -quality nursing management in the control of hospital infects . Methods High-quality nursing management in the control of hospital infects was implemented in our hospital from April 2012 , before and after implementation monitoring indexes were compared and analyzed including nosocomial infection rate , Hospital environment hygiene rate and medical staff hand hygiene compliance , in order to know about implementation effect .Results After implementing high-quality nurs-ing management, nosocomial infection rate drop 0.67%, hospital environment hygiene rate rose by 0.94%, medical staff hand hygiene compliance rose 8.92%.Conclusions High-quality nursing management can better control hospital infects and improve the quality of medical care . The Effect of Hand Hygiene Promotion in a University Hospital in China

Objective: Implementing a multimodal campaign could be resulted in a sustained increase in hand hygiene (HH) compliance rates. However, most studies of HH campaign have been conducted in the developed countries. This study was aimed to evaluate the effectiveness of hand hygiene campaign in a university hospital in China. Methods: During the period of the HH campaign, the WHO hand hygiene improvement strategy was implemented to improve hand hygiene compliance. We collected and evaluated the effect of hand hygiene campaign by the questionnaire of WHO hand hygiene observation, consumption of alcohol-based hand rub (AHR), liquid soap and gloves, and the quality of HH by adenosine triphosphate (ATP) test. Results: Of the 4,177 opportunities of hand hygiene evaluated, the HH compliance improved significantly from 24.2% to 41.0% after the intervention. Health care workers in the ICU showed higher hand hygiene compliance compared with those in other departments. In general, Nurses’ hand hygiene compliance was higher than those of physicians and surgeons. The consumption of AHR and the ATP pass rate increased after the intervention. Conclusion: Through the intervention, medical staff hand hygiene compliance was greatly improved from 24.2% to 41.0%. The WHO Hand Hygiene Strategy was also effective in hospital of China.

Does the architecture of hospital facilities influence nosocomial infection rates? A systematic review.

To review the evidence regarding the effects of interventions to improve hospital design and construction on the occurrence of nosocomial infections. Systematic review of experimental and non-experimental, architectural intervention studies in intensive care units (ICUs), surgical departments, isolation units, and hospitals in general. The studies dated from 1975, and were in English, French, German, Italian, and Spanish. Regardless of format, the studies were identified through seven medical databases, reference lists, and expert consultation. One hundred seventy-eight scientific articles were identified; however, none of these described a meta-analysis, systematic review, or randomized, controlled trial. Most of the articles were categorized at the lowest level of evidence (expert judgment or consensus statements). Only 17 described completed concurrent or historical cohort studies matching the inclusion criteria (ICUs, 9; surgical departments, 4; isolation units, 2; hospitals in general, 2). The interventions generally included a move to other premises or renovation. However, in many studies, the staff-to-patient ratio was also improved. Some studies showed lower infection rates after intervention, but this finding cannot be generalized because of confounding and frequently small study populations. The lack of stringent evidence linking hospital design and construction with the prevention of nosocomial infection is partly attributable to the multifactorial nature of these infections, and some improvement will be seen if basic conditions such as the availability of sufficient space, isolation capacity, and facilities for handwashing are met. However, to our knowledge, other factors, especially the improper hand hygiene of medical staff, have greater impact.
